Waiver and Release of Liability for Pixie Trixie Playhouse
This Waiver and Release of Liability (“Waiver”) applies to all activities and services offered at Pixie Trixie Playhouse, operated by Pixie Trixie. (“we”, “our”, “us”). By submitting this Waiver, parents, guardians, caregivers, helpers, legal representatives, or any accompanying adult (“You” or “you” or “Adult”) of participants, including children, wards, or students (“Participants”), agree to the terms and conditions herein. This Waiver must be submitted prior to participation in any activities (“Activities”) provided by Pixie Trixie Playhouse (“Playhouse”).
You confirm your agreement to comply with the Playhouse’s House Rules and any instructions from our staff while on the premises.
By submitting this Waiver, you confirm your understanding and acceptance of the terms, and you agree to hold Pixie Trixie Playhouse and its representatives harmless from any and all claims related to your or the Participants' use of the Playhouse’s Activities and facilities.
Warranties and Acknowledgments:

You warrant that you are over 18 years of age and are either the parent or guardian of Participants under 12 years old, or have the authority to accept this Waiver on their behalf.

You confirm that neither you nor the Participants have any medical conditions that would increase the risk of injury during Activities.

Participation in Activities and use of Playhouse facilities are at your and the Participants' own risk. Injuries may include, but are not limited to, cuts, bruises, falls, broken bones, sprains, strains, concussions, or other serious injuries.

Pixie Trixie Playhouse has taken reasonable steps to ensure the safety and proper maintenance of its facilities, but you understand and accept the inherent risks involved.

You accept full responsibility for the safety and supervision of any Participant under your care and agree to replace or reimburse Pixie Trixie Playhouse for any damages or losses caused by you or Participants.
Assumption of Risk and Release:

You agree that Pixie Trixie Playhouse and its employees, directors, and agents are not liable for any property damage, personal injury, sickness, or death that may occur as a result of participation in Activities, whether caused by negligence or otherwise.

You release and discharge Pixie Trixie Playhouse from any claims, actions, damages, or liabilities arising out of your or the Participants’ use of the Playhouse, its Activities, or related services, to the fullest extent permitted by law.

You acknowledge that the Activities require a moderate level of fitness, and Participants should only take part if physically capable.
Supervision and Conduct:

You are solely responsible for supervising Participants in your care at all times. Pixie Trixie Playhouse staff will not provide supervision for you or the Participants.

You agree to ensure that Participants conduct themselves safely and respectfully, without endangering or discomforting others.

Non-slip socks provided by Pixie Trixie Playhouse must be worn by all Participants during Activities.

You agree to adhere to all signs, notices, and instructions provided by Pixie Trixie Playhouse and acknowledge that failure to comply may result in suspension or removal from the premises without refund.
Additional Conditions:

You understand that outside food and beverages are not permitted within Pixie Trixie Playhouse premises. The Playhouse is not liable for any issues arising from the consumption of such items.

You consent to the use of CCTV monitoring throughout the premises for safety and security purposes.

You acknowledge and agree that Pixie Trixie Playhouse may collect, use, and disclose personal information in accordance with its Privacy Policy.
Legal Exclusions:

This Waiver does not exclude or limit any legal rights or remedies you may have under Singapore law, including any rights that cannot be excluded by law.

Pixie Trixie Playhouse does not exclude liability for death or personal injury resulting from its negligence or that of its employees or agents.
Pixie Trixie Playhouse Skin Allergy Liability Waiver
This waiver (“Waiver”) relates to the use of beauty and skincare products provided by Pixie Trixie Playhouse (“we,” “us,” “our”) during activities, including but not limited to DIY facials, makeup, manicures, pedicures, and other related services (“Activities”).
By submitting this Waiver, you (“Parent/Guardian”) consent to the use of these products on behalf of your child/ward (“Participant”) and acknowledge the following terms and conditions:
Risk Acknowledgment:
You acknowledge and understand that while the products used in the Activities are designed to be child-friendly, hypoallergenic, and safe for general use, there is still a possibility of allergic reactions or skin sensitivities due to individual differences.
Assumption of Risk:
You voluntarily accept all risks associated with your child/ward’s participation in the Activities, including potential allergic reactions or skin sensitivities. It is your responsibility to inform Pixie Trixie Playhouse staff of any known allergies, sensitivities, or medical conditions that could impact your child/ward’s experience.
Release of Liability:
You agree to release, waive, and discharge Pixie Trixie Playhouse, including its employees, agents, and representatives, from any and all claims, liabilities, or causes of action that may arise from allergic reactions or skin conditions resulting from the use of Pixie Trixie Playhouse products during the Activities.
Indemnification:
You agree to indemnify and hold harmless Pixie Trixie Playhouse, its employees, agents, and representatives from any claims, liabilities, damages, or expenses (including legal fees) that may result from any allergic reactions or adverse effects experienced by your child/ward as a result of participating in the Activities.
Emergency Medical Care:
In the event of an allergic reaction or medical emergency, you authorize Pixie Trixie Playhouse staff to administer basic first aid and, if necessary, to seek emergency medical treatment for your child/ward. You understand that any costs incurred for such medical treatment are your responsibility.
Acknowledgment and Agreement:
You confirm that you have read and fully understand the content of this Waiver. You acknowledge that you are voluntarily waiving certain legal rights, including the right to sue, and that you agree to these terms on behalf of your child/ward.
